Medical Nutrition Therapy

Medical Nutrition Therapy (MNT) is mandated by state law for residents in long-term care and skilled nursing facilities. This therapy involves nutrition-focused treatment provided by a registered dietitian nutritionist, known as an RD, or LDN. MNT, often referred to as "clinical nutrition," aims to prevent, diagnose, and handle certain health conditions. These conditions can include chronic kidney disease, heart disease, diabetes, malnutrition, digestive disorders, and other conditions that might need special nutrition support like tube feeding.

Weight Management

Weight management involves monitoring residents for major changes in their weight and taking action when needed. These changes can be either weight loss or weight gain. Every major change needs to be assessed to find out the reason, and a suitable plan will be decided by the Dietitian after discussing with the whole team. If the assessment shows malnutrition, the MDS team will be informed to update the CMS diagnosis code to make sure the facility gets the right reimbursement for any interventions used to address the diagnosis.

Standards of Care

All long-term care facilities must hold a weekly Standards of Care (SOC) meeting, sometimes referred to as a risk meeting. This meeting aims to identify high-risk residents based on specific criteria and assess them weekly to evaluate staff performance in providing proper care. SOC criteria include significant weight changes, recent falls or fall risks, wounds, and addressing any concerns from residents or their families. Weekly evaluations using SOC criteria ensure that your facility meets national minimum standards, provides the best care for your residents, and reassures families.
